Required Documents

For the Aeries AIR Online Enrollment process, you will need the following documents in a digital format: (PDF or photo of documents will be accepted.)

• Parent/Legal Guardian I.D.
• Proof of Age: Certified Birth Certificate, Passport, or Baptism Certificate
• Immunization Records:
  • The patient's yellow immunization card OR
  • Printout from patient portal showing dates with physician/clinic stamp or info, patient name, and birthdate
• Residency Verification/Declaration Form with TWO district-approved proofs of residency (Please review the Residency Requirements below)
• K-TK Enrollment Survey
• Additional information may be requested (e.g. 504 plan, IEP)

The K-TK Enrollment Survey is not required for preschool registration.

Intra-district transfers are not available for preschool students.
